ADMISSIONS Please, follow the next steps: 1.- Fill out Our Lady of Guadalupe School application by clicking this link Admissions online. 2.- The link will take you to the school's admissions page. Then, click on "create an account". It will direct you to FACTS portal. 3. Upon verification of your email address, you may log in to your account and proceed with creating a New Student Application for your child. A non-refundable fee $20.00 must be submitted with each application. You will receive a phone call to meet with the school Principal to join us one school day, a tour or answer any questions you may have. ENROLLMENT 1. You will be directed to FACTS Portal by clicking this linkFACTS Portal-Enrollment 2. Complete all the sections and upload the following documents to complete the process:
Child's birth certificate
Baptismal certificate
Immunization records
A non-refundable registration fee will be taken. Pre-K $250.00 per student
